To change the name on your account and bill, you will need to submit a new move in request. Your service will not be interrupted as the purpose is only to change service responsibility.

 2. Social Security Number is the same, but name has been changed due to:

• Marriage
• Divorce
• Legal Name Change
• Typo in Account Name

Please email the following documentation to the appropriate email address below.

• A copy of your ID and/or SSN, court order, or supporting document with name change
• Previous name on account
• New name
• Contract Account Number
