SCE Establishment Forms
Chapter 3 of the Direct Access ESP Handbook (PDF) contains the forms needed for ESP establishment with SCE to serve Direct Access customers within the SCE service territory. The Direct Access ESP Handbook provides detailed explanations for completion and how and when to use each form.

- The three most commonly enrollment used forms are described and provided below
CISR-S Forms
The CISR-S forms (PDF) are used to obtain customer information from SCE. Here you will find form instructions and additional information about customer authorization (PDF).
Phase IV: Six-Month Advance Notice to Transfer to Direct Access Service (Six-Month Notice) Form (PDF) and Process
Six-Month Notice forms will be accepted on a first-come, first-served basis beginning at 9:00 am (PST) on January 13, 2012 until the 2013 load cap is reached. All Six-Month Notice forms received before 9:00 AM (PST) on January 13, 2012 will be rejected.
- Participation is limited to retail non-residential customers
- Residential customers on bundled service are excluded
- Existing DA residential customers are allowed to remain on DA service
- Six-Month Notice forms will be accepted via e-mail and facsimile only

- Six-Month Notice forms (PDF) and Service Account spreadsheets should be attached to each e-mail in a password protected zip file to expedite system processing and minimize transmission delays
- Service Account spreadsheets should be sent as actual Excel files (.xls) as opposed to PDF copies of Excel spreadsheets
- Additional data rows should be added to accommodate additional service accounts as needed
- Customers should utilize the pre-formatted Excel template by clicking on the link in the right hand column of this web page: Six-Month Notice to DA — SA List Template (XLS)
- Based on each submitter's system requirements, the password for each password protected zip file submitted via e-mail should be either "123456" or "12345678"
- Six-Month Notices must be submitted via e-mail or facsimile only (see Six-Month Advance Notice to Transfer to Direct Access Service Form Submission at the bottom of this page)
- Customers should refer to the "Sample Six-Month Advance Notice to Return to DA (DOC)" document by clicking on the link in the right hand column of this page
- If deficiencies are reported to submitter and not resolved within 2 business days, the Six-Month Notice will be rejected
- Service Account spreadsheets should be sent as actual Excel files (.xls) as opposed to PDF copies of Excel spreadsheets
- An electronic Acknowledgement of Receipt will be returned to the submitter of each Six-Month Notice form received
- Six-Month Notices should be submitted only once to reduce e-mail congestion delays and expedite processing of submissions and status notifications
- All service accounts listed on a Six-Month Notice form must be for the same customer of record indicated on the Six-Month Notice form
- Service accounts with a Federal Tax ID (FTID) number that does not match the FTID number for the customer of record indicated on the Six-Month Notice form will be rejected
- Six-Month Notice forms will be time and date stamped to determine precedence
- Six-Month Notice forms found to have a deficiency, such as incorrect service account number and missing or illegible information, may be accepted on the condition that the deficiency is corrected within 2 business days after receiving notification of the deficiency
- Service Account numbers appear on the customer's SCE bill formatted as "3-010-2010-20"
- Service Account numbers always begin with "30" followed by eight digits
- Service Acocunt numbers should be submitted to SCE without the separating dashes (-) "3010201020"
- SCE will process Six-Month Notice forms within 30 business days of receipt
- Submitters of Six-Month Notices will be notified electronically whether their Six-Month Notice form has been accepted or rejected
- Acceptance notifications will include switching process information and DASR submission deadline date
- Customers are responsible for providing this information to their ESP
- Individual e-mail or facsimile submissions containing more than one Six-Month Notice form will be rejected
- Accepted Six-Month Notice forms will be switched to DA starting no later than January 2, 2013, provided the six-month advance notice period is satisfied and DASRs are submitted in a timely manner
- No Wait List will be established
- Should additional load become available aftter the close of Phase IV, SCE will post a status change on SCE.com indicating that Six-Month Notices are being accepted
- Customers are allowed three calendar days to rescind or withdraw their Six-Month Notice
- Rule 22.1 — Switching Exemption Guidelines (PDF) applies to all Six-Month Notices received by SCE during the 2013 Six-Month Notice submission period
- If a DASR is not received by the final date specified in the acceptance notification, the associated service accounts will be transferred to Transitional Bundled Service (TBS) for sixty days
- If a DASR is not received by the end of the sixty day TBS period, the Six-Month Notice will be cancelled
- After the six-month advance notice is canceled, the account will no longer be eligible for DA service
- The account will continue to bill on the Transitional Bundled Service (TBS) spot market price for an additional six months
- At the end of the six-month TBS period, the account will begin bundled service for an 18-month Bundled Portfolio Service (BPS) commitment term

Bundled Portfolio Service (BPS)
DA customers choosing to return to bundled service for a term longer than 60 days must provide to SCE a Six-Month Advance Notice to Return to Bundled Portfolio Service (PDF) prior to becoming eligible for BPS service. Customers have three business days after SCE receives their six-month advance notice to rescind the notice and remain on DA service. Customers electing BPS service make an 18-month commitment and will not be eligible to return to DA service until their 18-month minimum period has been completed. Customers returning to BPS service continue to be responsible for any Direct Access Cost Responsibility Surcharge (DA CRS) under-collection balance incurred during the period they were taking DA service. At the end of the initial 18-month term, customers will have the option to return to DA service or remain on bundled service.
Direct Access (DA) Service
Valid for customer use on or after July 16, 2010
Customers who have fulfilled their 18-month BPS commitment have the option of transferring to DA service. Customers electing to transfer to DA service after the end of the 18-month BPS commitment period are required to provide to SCE six months advance notice using the Six-Month Advance Notice to Transfer to Direct Access form (PDF). Customers have three business days after SCE receives their six-month advance notice to rescind the notice and remain on bundled service. SCE will provide the customer a written confirmation and necessary switching process information within 10 business days of the customer's notice, including the final deadline for SCE to receive a Direct Access Service Request (DASR) from their Electric Service Provider (ESP) to return to DA service. Customers are responsible for providing this information to their ESP.
